If you have been awarded state financial aid from the State of Washington, your designation on this online form will document your choice for funds to be applied directly into your student account or to be given directly to you in the form of a check. A state grant or scholarship recipient has the following two choices:

NWIC Student Account: Choosing to have the funds deposited to your student account permits the aid to be automatically credited toward expenses you may owe to the institution. This option allows faster processing of any refunds of financial aid proceeds that may be due you.

Directly to Student: Choosing to have state financial aid funds given directly to you means that state student aid will not be automatically available to pay for tuition and fees. You are responsible for using the funds to pay for educational expenses including any expense you owe to the institution.

How do you want state funding delivered?

I understand that the aid offered is for a full-time course load (12 credits) unless otherwise specified on the award letter. I agree that if I withdraw or cease to attend or carry the required number of credits, I will notify the Student Financial Services office and, if necessary, arrange for the repayment of aid advanced to me for which I am no longer eligible.

I understand that I am required to attend classes to earn financial aid. If I withdraw from classes during the quarter or do not earn any credits, I may be required to repay all or part of my aid received.

I understand that I must make satisfactory academic progress to receive Financial Aid. Financial Aid may be canceled for failure to make satisfactory progress. This means that I must complete my degree within an established time frame, maintain a minimum cumulative grade point average, and successfully complete the minimum number of credits each quarter for which aid is received. I understand that I am responsible for knowing the information contained in the Satisfactory Academic Progress Policy.

I must enroll in an eligible program of study and the classes I take must be required for that program.

I understand that the offer of aid is dependent upon federal and state allocations to the college. Insufficient funds may result in a reduction in the amount of aid or a change in the type of aid offered to me.

I understand that the award is based on information I have provided about my financial aid status. I agree to report to the Financial Aid Office any changes in my financial situation, including, but not limited to: changes in marital status, address, Washington State residency status, any additional income, scholarships, or other funds I may receive after returning the award letter. I understand that these changes may result in a reduction of awards and/or require repayments of aid already received.

If I am offered an aid package but chose not to enroll for one or more quarters during the academic year, I will inform the financial aid office of my intent to return or I will forgo any non-Pell awards in following quarters.

If I deliberately falsify, misrepresent, or fail to fully disclose information on my financial aid application, I can be terminated from financial aid. If I receive funds based on misrepresented information, I will be required to repay the funds and may be reported to the US Department of Education, and/or other financial aid governing bodies, for possible fraud.

I declare that I will use all funds I receive (federal and state) for the expenses associated with cost of attendance at Northwest Indian College. If any information on this online form changes, I will submit a new form.

Do you agree to the General Conditions of Award?

If I qualify for any Washington State Aid, including the Washington College Grant, State Work Study, College Bound Scholarship, or Passport to College/Careers Scholarship, I must adhere to the following conditions as provided by the Washington Student Achievement Council:

1. You must meet the requirements for Washington State residency.
2. You do not owe a repayment to any federal or state grant or scholarship nor are you in default on a state or federal student loan.
3. You must be enrolled in an eligible program and not be pursuing a degree in theology.
4. If you hold a bachelor’s degree or the foreign equivalent, you are not eligible to receive state grant funds.
5. If you do not attend or if you withdraw from your classes, you may owe a repayment of all or part of any state funds you have received.
6. You must maintain the academic progress standards established by your institution in order to receive additional state aid.
7. There could be other circumstances that would require a repayment or reduction in your current award amounts.
8. You must meet all eligibility requirements for the state aid program(s) awarded.
9. The offer of this financial assistance is subject to, and conditioned upon, the availability of funds. Washington Student Achievement Council (WSAC) and the institution through which the grant, scholarship, or work study is awarded, reserve the right to withdraw, reduce, or modify the awards due to funding limitations or due to changes in circumstances which affect your eligibility for the program(s).
10. If you fail to cash your check containing state funds or pickup any remaining funds by the close of the academic year, the funds shall be returned to the program at WSAC and treated as funds declined by you.

You may choose to voluntarily make financial contributions to the Washington Student Achievement Council in recognition of the assistance that you received.
